This is burned in my brain, thought about it all day today.

One concern I have is getting lost. There's no lines in the street to follow (thankfully), so I'm not sure how you'd find your friend or the doctor to move further through the game. The dev's knew where to go, but how will we?
That said, I'd rather be utterly lost 95% of the time than just follow a line. It's why I always liked FarCry and JustCause, love roaming around breaking things.

This really has the chance to take gaming forward another step. This could be the next Bioshock, TF2, L4D, CoD4:MW, Portal 2, Overwatch. A game you'll look back and say, "I was there".

The new tech with ray tracing and all that is really doubtful to be a big deal, is my guess.
I imagine it will be like a roof-top restaurant, cool view, crappy food.
I'm much more interested in fun interesting game-play than fireball reflection.

I really really hope this lives up to expectations, if it's even half as cool as I think it's going to be then it's gonna be awesome.

I put like 140 hours into this developer's last game - Witcher 3. It was an insanely huge open world game and there was never a moment where I didn't know where to go or what to do next. IMO, these guys are masters of open world game play. Give Witcher 3 a try (I think it's on sale at GOG right now) and you'll see what I mean.
